Forms

We have our patient forms available for you. It really helps us to better facilitate your appointment if you complete all required forms prior to arrival. You can download, print, and bring the required forms with you.

Arrival Time

For your child's first appointment, please arrive 15 minutes early to complete paperwork. Please bring all relevant paperwork from your previous pediatrician, including immunization records and Lab test results.

Insurance

Bring identification, such as a driver's license, and current copy of the insurance card that covers your child.

Billing Policies

We accept most insurance carriers (Medicaid, Wellcare, Peachstate, Caresource, Amerigroup, Aetna, Blue Cross Blue Shield,Beachstreet, Cigna, Coventry, First Health, GHS, Humana, multiplan). If you have any questions about coverage of your specific insurance, please call our office. For your convenience, we accept cash, personal checks, Visa and Mastercard.
